  Welcome to the registration page.
  To register, and pay the materials fee of $50 that covers the costs associated with Confirmation Prep ($40 to the parish and $9.99 for the Strengths Explorer assessment. Strength Explorers can be found in task two. ).

  To complete this form, you will need all the information listed on the Candidate’sBaptism Certificate.•
  If the Candidate was baptized at another parish, we will need a Certificate with Parish raised seal complete with all notations for prior sacraments. All Candidates must have received Baptism and First Communion prior to Confirmation. To obtain this, simply call the church of Baptism and request an updated Baptismal Certificate with notations. You can have them mail it directly to:

  If your candidate was baptized in a Catholic parish that has been closed or has been combined during the On Mission process (i.e. Good Samaritan is now St. Luke the Evangelist), the Diocesan Archives Office now houses those sacramental records. We will obtain those records for you.
  
  If your candidate was baptized in another Christian Denomination and converted to Catholicism, you will need to contact the parish where your candidate made a Profession of Faith to obtain a verification form from them, not from the church of Baptism.
  We need to see both sides of the document.
  The parish will obtain records from within the diocese that are now at the Diocesan Archives.
